Privacy notice for this website

This notice covers perivantahealth.com — the marketing website. How the Perivanta platform handles clinical and operational data for customers is a separate, contractual matter governed by business associate agreements, described on our security page.

What we collect here

• What you send us. If you submit the design partner form or email us, we receive what you wrote: your name, work email, role, organization, and message.
• Basic technical logs. Like almost every website, our hosting infrastructure records standard request logs (IP address, browser type, pages requested) used for security and reliability.

What we do with it

We use what you send us to respond to you and to evaluate design partner fit. That’s it. We don’t sell your information, we don’t share it with advertisers, and submitting a form does not enroll you in a newsletter or automated email sequence.

What we don’t do

• No sale or rental of personal information, to anyone.
• No advertising trackers on this site.
• No patient information: this website is not a channel for PHI, and you shouldn’t submit any here.

Retention and your choices

We keep inquiry records for as long as the conversation is live or reasonably could be. If you’d like your information corrected or deleted, email scott@agenticmediasolutions.com and we’ll take care of it.
